You can download latest version of Action Form from www.avatar-soft.ro. While on that page, you can also check the changelog to see what's new.

Action Form is compatible with DotNetNuke module definition and therefore can be installed via Host > Module Definitions > Install New Module. On successful download, it will appear in the list of modules in Control Panel box.

Instantiate Action Form an a page and container as you normally would with any other module. Once it appears on the page, locate the management screens (they can be accessed from Module Actions or from “Manage Form” or “Form Reports” icons in the right).
